In proceeding with making a contribution, donation, grant, transfer, or gift to Committee to Save Senior Services, you certify that: in the case of an individual donor, you are a U.S. citizen or U.S. national and the funds are your own; and in the case of a non-individual donor, the donating entity is organized under the laws of a state of the U.S. and has its principal place of business in the U.S. and that the underlying sources of the funds are from U.S citizens or nationals or entities organized under the laws of a state of the U.S with their principal place of business in the U.S. Under Ohio law donations by non-citizens with permanent U.S. resident status (green card holders) are not permitted.
